Registration is on the ground floor. Bring your ID, insurance card and any previous reports.
This form is not for emergencies. If you have chest pain, difficulty breathing, heavy bleeding or sudden weakness, call +1 800 123 4567 or come directly to the emergency entrance.
Booking Questions
Yes. Call the number on your confirmation message at least four hours before your slot and we will move it free of charge. Repeated no-shows may affect priority booking.
Not for most outpatient clinics. Some sub-specialty services — genetics, transplant assessment and complex pain — do require a referral from your primary doctor.
Only if your appointment includes fasting bloods or an abdominal scan. Your confirmation message will say so explicitly — if it does not mention fasting, eat normally.
One companion is welcome in outpatient clinics, and we encourage it for consultations where a treatment decision is likely. Ward visiting hours are 11:00–13:00 and 17:00–20:00.
